Product Model Identification Service
PARTS ARE MODEL-SPECIFIC
Identifying your product model is very important as most replacement parts are model-specific, and, often serial number specific. In order to assure you get the correct part, avoid costly returns, and return to safe mobility as quickly as possible, we need the brand and model number of your item.
WHERE TO LOOK
Model numbers (also known as: Product Code, Ref. No., Item#, etc.) are located on a Product ID Label or Metal Tag. For Rolling Walkers, Wheelchairs or Transport Chairs the ID Labels are usually located on one of the lower metal-frame members. (for Nova walkers it's under the seat on the seat/frame cross bar). Model numbers for other products are usually found on a metal Product ID Tag.
CAN'T FIND THE MODEL NUMBER?
In the event you cannot identify your product model number, we offer our Product Model Identification Service. Once you purchase this service and provide the specified product photos (see instructions below), we will make a good faith effort to identify your product so that you may purchacse the correct replacement parts.
NON-REFUNDABLE SERVICE
If our ID Service is successful - you will receive a 10% discount for purchases associated with the identified product. However, there is no guarantee the ID Service will be successful, even with our best efforts. Therefore, the purchase of this service is non-refundable.*
HOW TO ORDER PRODUCT ID SERVICE:
1. Go to Top of Page and Fill in Required Fields.
2. Add to Cart.
3. Place the order.
4. You will receive via email an Order Confirmation. Please Keep your Order Number.
5. Email 3 Photos only... • Full Side View • Full Front View • Product ID Label (legible) to: rollator777@gmail.com. Please include your Order Number in the email.
6. Wait for a response. (Responses typically take 48 to 72 hours)
WHEN PRODUCT MODEL ID IS SUCCESSFUL
When the product is identified, we will...
1. Notify you via email with your Product ID.
2. Provide a link to that product for making your purchase.
3. Include a 10% Discount Code toward purchases associated with the identified product.
* In the event the identification of your product is unsuccessful, you will notified of that via email.
